⚠️ BETA NOTICE: CMgmt Hub is currently in beta. This app is a personal caregiver organization tool and is NOT intended for storing clinical, medical, or protected health information (PHI). Please do not enter diagnoses, test results, medication dosages, provider instructions, or any sensitive clinical data.

1. Introduction

Welcome to CMgmt Hub, a personal caregiver organization application operated by FMD Business Services LLC ("we," "us," or "our"). This Privacy Policy explains how we collect, use, disclose, and protect your information when you use our mobile application and related services (collectively, the "App").

By using CMgmt Hub, you agree to the collection and use of information in accordance with this policy. If you do not agree with the terms of this Privacy Policy, please do not use the App.

2. Information We Collect

2.1 Information You Provide

When you create an account and use the App, you may provide:

• First name or nickname (for account identification)

• Email address (for account login and communication)

• Patient nicknames or first names (for organizing care records)

• Appointment details such as type, date, care provider name, and location

• Medication names and scheduling information (no dosages)

• Transportation details including pickup and drop-off locations

• General notes and activity updates

• Task lists and wellness check-in data

2.2 Information We Do Not Collect

CMgmt Hub is designed as a personal organization tool, NOT a clinical or medical records system. We strongly discourage and our app is not designed to store:

• Medical diagnoses or clinical conditions

• Medication dosages or prescription details

• Test results or lab reports

• Provider clinical instructions or treatment plans

• Social Security numbers or government identification

• Financial account information or payment details

• Insurance information

2.3 Automatically Collected Information

When you use the App, we may automatically collect:

• Device type and operating system

• App usage data and feature interactions

• Error logs and crash reports

• General location data (not precise GPS tracking)

3. How We Use Your Information

We use the information we collect to:

• Provide, maintain, and improve the App and its features

• Create and manage your user account

• Enable patient data organization and care coordination features

• Send account-related notifications and updates

• Respond to your support requests and inquiries

• Analyze usage patterns to improve user experience

• Enforce our Terms of Service and legal obligations

4. Data Storage and Security

CMgmt Hub is built on Base44, a no-code application platform. Your data is stored in Base44’s infrastructure. We take reasonable technical measures to protect your information, including:

• User-level data separation so you only see your own data

• Secure account authentication via email

• HTTPS encryption for data in transit

However, please be aware that CMgmt Hub is currently in beta and is NOT certified as HIPAA-compliant. We are working toward full security compliance in future releases. For this reason, we strongly advise users NOT to store sensitive medical or clinical information in the App at this time.

5. Data Sharing and Disclosure

We do not sell, trade, or rent your personal information to third parties. We may share your information only in the following circumstances:

• With service providers (such as Base44) who assist in operating the App under confidentiality obligations

• When required by law, court order, or governmental authority

• To protect the rights, property, or safety of FMD Business Services LLC, our users, or the public

• In connection with a business merger, acquisition, or sale of assets (with advance notice to users)

6. Data Retention

We retain your information for as long as your account is active or as needed to provide services. If you delete your account, we will delete or anonymize your personal data within 30 days, unless retention is required by law.

Activity Updates are subject to tier-based retention limits. Free tier users can access the last 3 months of activity history. Paid tier users retain unlimited activity history.

7. Your Rights and Choices

You have the right to:

• Access the personal information we hold about you

• Request correction of inaccurate information

• Request deletion of your account and associated data

• Opt out of non-essential communications

• Export your data before account deletion

8. Children's Privacy

CMgmt Hub is not intended for use by individuals under the age of 13. We do not knowingly collect personal information from children under 13. If you believe a child under 13 has provided us with personal information, please contact us at fmdbusinessservices@gmail.com and we will promptly delete such information.

9. Third-Party Services

The App may integrate with or link to third-party services. This Privacy Policy does not apply to those third-party services, and we are not responsible for their privacy practices. We encourage you to review the privacy policies of any third-party services you use in connection with the App.

10. Changes to This Privacy Policy

We may update this Privacy Policy from time to time. We will notify you of any material changes by posting the new policy within the App and updating the effective date. Your continued use of the App after any changes constitutes acceptance of the updated policy.
